【提出書類と発送について】
1. 提出書類の準備
以下より必要書類を印刷し、必要事項をご記入ください。
また、出願申請完了後、マイページより「入学願書」「受験票」「宛名ラベル」を印刷してください。
2. 郵送手順
角形2号封筒をご用意いただき（願書用の封筒でも代用可）、印刷した宛名ラベルを貼り付けてください。
すべての提出書類を同封のうえ、締切日までに郵送にてご提出ください。
Required Documents and Mailing Instructions
1. Prepare the Required Documents
Print the required application documents below and complete all required fields.
After completing your application, print the Application Form, Examination Admission Ticket, and Address Label from My Page.
2. Mailing Procedure
Prepare a No. 2 Kaku envelope(or use the envelope included in the university's application packet), and affix the printed address label to the envelope.
Enclose all required application documents and mail them so that they arrive by the application deadline.
封筒(角形２号封筒)は各自でご準備ください。(封筒や書類は願書でも代用可)
出願申請完了後、マイページから宛名ラベルを印刷し、
封筒に宛名ラベルを貼付のうえ、必要書類を同封し、
各試験締切日の2営業日後13時までに本学へ到着するよう、簡易書留にて郵送してください。
Please prepare your own No. 2 Kaku envelope. (The envelope and forms included in the university's application packet may also be used.)
Affix the address label to the envelope, enclose all required application documents, and send them by registered mail so that they arrive at the university by 1:00 p.m. on the second business day after the application deadline for each entrance examination.

推薦書Public Recommendation Letter
公募推薦選抜入試において提出が必要となります。
出身学校長の記入が必要となります。
高等学校卒業程度認定試験合格者および合格見込み者は、「合格（見込）証明書」「成績（見込）証明書」の提出が必要です。
スポーツ推薦入試について必要な書類は、高等学校野球部顧問もしくは監督へ詳細を確認してください。
Required for applicants to the Public Recommendation-Based Selection.
Must be completed by the principal of your current or former school.
Applicants who have passed, or are expected to pass, the Upper Secondary School Equivalency Examination must submit a Certificate of Passing (or Expected Passing) and an Academic Transcript (or Expected Academic Transcript).
For the required documents for the Sports Recommendation Entrance Examination, please contact your high school baseball club advisor or coach for details.

履歴書Resume
留学生入試、渡日生入試において提出が必要となります。
学歴欄、職歴欄は正確に記入してください。（小学校から記入）
写真は入学願書と同じものを使用してください。
This document is required for the International Student Admission and Overseas Applicant Admission.
Please provide accurate information in the Educational Background and Employment History sections. (Include your educational history starting from elementary school.)
Use the same photograph as the one submitted with your application form.
認定書Certificate
渡日生入試において提出が必要となります。
出願資格（2）に該当する者は、在籍高等学校長が発行した認定書を提出してください。
出願資格（2）満18歳以上の者で、外国の学校に在学した後、日本に帰国もしくは渡日して12年の教育課程のいずれかに入学し修了した者、
および2027年3月修了見込みの者
This document is required for the Overseas Applicant Admission.
Applicants who meet Application Eligibility (2) must submit a certificate issued by the principal of their current high school.
Application Eligibility (2): Applicants who are 18 years of age or older, who studied at a school outside Japan and then returned to or came to Japan, enrolled in and completed a 12-year education program, or are expected to complete such a program by March 2027.
